Forthefirsttimeinhistory,morepeopleliveincitiesandtownsthaninruralareas.Inaparalleltrend,theburdenofworldpovertyisalsoshiftingfromsparselypopulatedruralareastodenselypopulatedcities.Bymid-century,urbandwellerswillcountforsevenoutofeverytenpeople.Mostofthisexplosivegrowthisoccurringindevelopingcountries.Rapid,unplannedurbanizationisexpandingslumsandinformalsettlementsandmunicipalauthoritiesarestrugglingtocope.
城鎮(zhèn)人口有史以來第一次超過農村人口。與此平行的另一趨勢是，世界貧窮的負擔也從人口稀少的農村地區(qū)轉移到了人口密集的城市。到本世紀中葉，城鎮(zhèn)居民將占總人口的十分之七。這種爆炸式的增長大都發(fā)生在發(fā)展中國家。快速而無計劃的城市化造成越來越多的貧民窟和非正規(guī)住區(qū)，市政當局正窮于應對。
Thedisparityinpeople'sincome,opportunities,livingconditionsandaccetoservicesismostvividlyreflectedbythemirrorofpublichealth.Thethreatsarenumerous:inadequatesanitationandrefusecollection;industrialandtrafficpollution;infectiousdiseasesthatthriveonsqualorandcrowdedconditions;highratesoftobaccouse;physicalinactivity;unhealthydiets;crime,violenceandtheuseofharmfulsubstances.
人們的收入、機會、生活條件和可獲服務極為不均，這最為清晰地反映于公共衛(wèi)生方面。面臨的威脅難以勝數(shù)：環(huán)衛(wèi)設施和垃圾清收不足；工業(yè)和交通污染；在骯臟和擁擠的情況下容易滋生的傳染�。粺熋癖姸�；不愛鍛煉身體；不健康的飲食；犯罪、暴力和使用有害物質。
Toalargeextenttheseproblemsliebeyondthedirectcontrolofthehealthsector.Improvingurbanhealththereforerequiressoundpoliciesacroallareasofgovernmentandawareneamongallsectorsofsociety.ThebroadfamilyofUNagenciesandprogrammesisinvolvedinthiseffort:workingtoreduceairandnoisepollution,trafficcongestionandcrime;helpingtoimprovehousing,sanitationandfoodandwatersafety.
這些問題在很大程度上超出了衛(wèi)生部門的直接控制。因此，改善城市衛(wèi)生需要在政府工作各個領域實施健全的政策，需要社會所有部門都認識到這一問題。聯(lián)合國廣大的機構和方案參與了這項工作：努力減少空氣和噪音污染、交通擁堵及犯罪現(xiàn)象，幫助改善住房、環(huán)境衛(wèi)生以及食物和用水安全。

TheUniversalDeclarationofHumanRightsandtheInternationalCovenantonEconomic,SocialandCulturalRightsguaranteetherightofeveryonetoenjoythehighestattainablestandardofphysicalandmentalhealth,includingtomedicalcareandservices,withoutdiscriminationofanykind.
《世界人權宣言》和《經濟、社會、文化權利國際公約》都規(guī)定，人人享有可達到的最高標準身體和精神健康的權利，包括不受歧視享有醫(yī)療保健服務的權利。
TheconstitutionoftheWorldHealthOrganizationreferstothephysical,mentalandsocialaspectsofourwellbeing,whicharecloselylinked.Mentalillnessesseriouslyaffectourbodiesandoursocialrelationships,whilephysicalhealthproblems,especiallywhensevereandprotracted,canleadtosocialisolationandmentalillness.

Butmanycountriesfallfarshortintreatmentforthosesufferingwithmentalillne–eventhoughitisrelativelyinexpensiveandeasytodeliver.Thevastmajorityofpeoplewithmental,neurologicalandsubstanceusedisordersdonotreceiveeventhemostbasiccare.Yetsuchservicesareessentialifwearetoofferhopetosomeofthemostmarginalizedpeopleintheworld,especiallyindevelopingcountries,tolivetheirlivesindignity.
精神疾病的治療相對容易，成本也較為低廉。但是，許多國家卻遠遠沒有做好治療工作。絕大多數(shù)精神、神經和藥物濫用患者，甚至無法得到最基本的治療。然而，我們必須提供這種醫(yī)療服務，我們要給全世界、特別是發(fā)展中國家的最邊緣群體帶去有尊嚴生活的希望。
